the need is, why its and emergency, How
much is needed (Exact amount) The full
Name of person requiring help and
contact info. We must be able to speak
with person that is in need of Emergency
Help Directly. If that person is
incapacitated to the extent that they
cannot make the request in person then
the next available family member is
acceptable.
